Many adults with developmental disabilities have significant health conditions requiring nursing-level care. Our Registered Nursing Support program provides skilled clinical services that allow medically fragile or complex clients to live in community settings rather than medical facilities.

Our registered nurses work closely with physicians, families, and care teams to manage medical conditions, prevent complications, and promote optimal health while maintaining quality of life.

Who Needs RN-Level Support?

Registered nursing services are required for clients who:

• Have medical conditions requiring RN assessment and intervention
• Need procedures that only RNs can legally perform
• Require complex medication regimens with clinical monitoring
• Have unstable conditions needing frequent nursing evaluation
• Use medical equipment requiring RN-level management
• Are recovering from hospitalizations or surgeries

How RN Services Are Delivered

On-Site Nursing

Our RNs visit residential settings on scheduled shifts to complete assessments, perform procedures, administer medications, and monitor conditions.

On-Call Availability

For clients with unstable conditions, RN phone consultation is available 24/7 to guide care staff through urgent situations.

Medical Appointment Attendance

Nurses accompany clients to doctor visits, communicating health history, asking appropriate questions, and ensuring proper follow-through.

Care Plan Development

RNs create detailed nursing care plans that guide direct care staff in daily health management.

Regular Reviews

Health status is formally reviewed quarterly (or more often if needed) with updates to care plans as conditions change.

Working with LVN Services

Some clients benefit from a combination of RN and LVN (Licensed Vocational Nurse) support. Generally:

RNs handle:

• Initial assessments and care plans
• Complex or unstable conditions
• Procedures requiring RN licensure
• IV medications and central lines
• Clinical judgment situations

LVNs handle:

• Routine medication administration
• Vital sign monitoring per established parameters
• Glucose checks and insulin per physician orders
• Routine wound care and dressing changes
• Daily health monitoring

We coordinate RN and LVN services to provide cost-effective care that meets each client's needs.

Getting Started with RN Services

Step 1: Regional center or family contacts us regarding medical support needs.

Step 2: We review medical records and current health status.

Step 3: An RN completes a comprehensive nursing assessment.

Step 4: We develop a nursing care plan and determine service frequency.

Step 5: Regional center authorizes services through Service Code 744.

Step 6: RN services begin with close initial monitoring.
