Personal Attendant
As a Personal Attendant, you'll provide direct support to adults with developmental disabilities, helping them with daily living activities, community access, and personal goals. This is a rewarding role for compassionate individuals who want to make a hands-on difference.
- Assist with daily living activities and personal care
- Support community integration and social activities
- Help individuals work toward their IPP goals
- Provide transportation to appointments and activities
- Document progress and communicate with team members
Life Guide (SLS)
Life Guides work within our Supported Living Services program, providing comprehensive support to individuals living in their own homes. You'll be a trusted companion, helping with everything from daily routines to meaningful community participation.
- Provide in-home support for daily living
- Accompany individuals on community outings
- Support health and safety in the home
- Build trusting, respectful relationships
- Coordinate with Case Facilitators and families
ILS Coach
ILS Coaches work within our Independent Living Services program, teaching and reinforcing skills that help individuals live more independently. This role focuses on education, skill-building, and empowering individuals to reach their full potential.
- Teach independent living skills (cooking, budgeting, etc.)
- Develop and implement skill-building plans
- Support self-advocacy and decision-making
- Track progress toward individual goals
- Collaborate with Case Facilitators on service planning
Case Facilitator
Case Facilitators coordinate services for a caseload of individuals, overseeing their support teams and ensuring quality care. This leadership role is ideal for experienced professionals ready to take on more responsibility.
- Manage a caseload of up to 8 individuals
- Coordinate and supervise direct support staff
- Develop and monitor Individual Service Plans
- Communicate with families, Regional Centers, and other stakeholders
- Ensure compliance with DDS regulations
