Our Approach
At Congregate Connect, we understand that every individual is unique. That's why we take a person-centered approach to everything we do. We start by listening—to the individual, to their family, and to their support team—to understand what matters most to them.
From there, we build a support plan that reflects their goals, respects their preferences, and sets them up for success. Our highly trained staff provide consistent, reliable support while encouraging independence and self-determination.
We're proud to be licensed for 8 DDS service codes, allowing us to offer a comprehensive range of services under one roof. This means individuals and families don't have to coordinate between multiple providers—we can meet a wide variety of needs with our integrated team approach.
